What to do in an emergency
We have an open list and welcome requests for registration from patients living in or moving to the practice area.
You can register by bringing along your NHS medical card or completing a form available from reception or downloadable below, you will also need two forms of identification, one photographic the other proof of address. We will make a note on your records if you prefer to see a particular doctor.
We recommend that new patients undertake a health check with a practice nurse.

Students based at Norwich University of the Arts (NUA) can now register with us electronically. If you register with us electronically, we will ask NUA to confirm they have carried out an ID check with you at Enrolment. UK, European and International students can register with us.
At the start of term we will have a high number of forms to process, so there may be a delay of a few days in registering you. If you need to see a doctor urgently, please come in person to our practice at Oak Street, bringing ID with you, and we can register you in person to enable a doctor to see you more quickly.
Alternatively you can use Rouen Road Walk-In Centre in Norwich (www.norwichwalkincentre.co.uk/Walk-in-About-us.htm) for some types of medical help, even if you are not registered with any GP Practice.

You can then select the desired save location on your computer. Please be aware that clicking ‘Enable Editing’ will not allow you to fill in the form unless it is saved using the above method. Fill in this form electronically – it does not need to be printed or signed. We cannot read .Pages files (Mac compatible) so please ensure you return a .doc or .docx form.
This form is used by the National Health Service (NHS) for people registering with a new Medical Practice. You will need to print it, fill it in, then scan it to your computer.

SystmOnline – managing your appointments and care online
- SystmOnline makes booking appointments and requesting medication easy. Read more about it in the Information Leaflet below. In your NUA Registration Questionnaire you can request access to SystmOnline. You will then be sent a user name and password by email, then you can log in using the link below.

We do of course understand that times of stress and illness can lead to people being less tolerant or polite than usual. We are here to help at those times and will be as patient and tolerant as we can.
Whatever the reason may be, however, we do not tolerate any violent, aggressive or abusive behaviour to anyone working in or visiting the surgery. Patients who offend against this principle will normally be removed from our list.
If you change name, address or telephone number, please let our receptionists know by calling or writing to us. If you move outside the practice area you may need to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card.
Our Practice is Research Active
All NHS organisations are expected to participate and support health and care research. Conducting high-quality clinical research helps us to keep improving NHS care by finding out which treatments work best. Oak Street Medical Practice is part of a network of local practices participating in research activities under the banner of Clinical Research Network (CRN) Eastern. If you are asked about taking part in research, usually someone in the care team looking after you will contact you. People in your care team may look at your health records to check whether you are suitable to take part in a research study, before asking you whether you are interested or sending you a letter on behalf of the researcher. The Data Protection Act of 2018 and the General Data Protection Regulations (GDPR) requires that we tell you how we keep records about you and who has access to them. Nowadays we keep your clinical records on computer, although some earlier records are still on paper in the old folders. Some entries on patients’ records are made by members of staff, on behalf of the doctors and nurses. At times, the staff will need to look at the records either to pass on information such as test results to you, or for essential administrative purposes. We all, whether we are clinicians or staff, treat your records with great respect and apply strict rules of confidentiality.
As a patient, you have a right to see all the medical records we hold on you (except in certain special and unusual circumstances). If you wish to have access to your records please ask to speak to our Practice Manager who will make the arrangements. You will normally have a paper record and a computer record. You can see both and can ask for copies or prints to take away. There is a standard fee of £10 for having access and a charge per page for photocopying.
You now have the right to have a copy of any letter we send to other clinicians about you. If you would like this, then please let us know.

Provide a friendly, polite service.
Treat patient information with utmost confidence.
Make every effort to keep to appointment times but sometimes delays are unavoidable if the GP gets called out to an emergency or other serious problems arise. We will keep you informed if this happens.
Answer the telephone promptly. Inevitably there are times of high demand when it is difficult to get through. Please be paitent.
We ask our patients to:
- Be punctual for appointments.
- Cancel and appointment if unable to attend.
- Remember that an appointment is for one patient only.
- Request a repeat prescription 48 hours before it is needed.
- Request a home visit only if too ill to attend surgery or are housebound.
- Inform us of any change of name, address or phone number.
- Treat doctors and practice staff with courtesy.
